Requirements
Only some diseases need to be reported. Consult the Health Information Portal (HIP) to see an updated list of diseases that must be reported.
How
To report a disease on weekdays from 9 a.m. to 5 p.m., call (215) 685-6748. Outside of these hours, call (215) 686-4514 and ask for the on-call staff.
