CSHCN

Chil dren with special care needs (CSHCN) are those children who have or are at increased risk for chronic physical, emotional, behavioral and developmental health conditions. The medical and social needs of CSHCN often require a high degree of support. The Division of Maternal, Child and Family Health vigorously engages with issues that affect the lives of CSHCN and their families at both a ?systems? level and an individual level.

The Program Coordinator works with public and private service providers to facilitate and enhance ?system? collaboration among those serving CSHCN.

The Community Systems Coordinator works with hospital staff and families to ensure an additional layer of service and support to families of CSHCN.

The Special Needs Work Group provides a forum for information sharing and networking among families, health care and insurance providers, legal advocates and local, state and federal policy and planning agents who directly or indirectly influence care for CSHCN.
