
Our Health Intervention Program for Families (HIP) provides home visiting and other support to families with infants and children (0-21) who have special healthcare needs.
Children with special healthcare needs (CSHCN) are children who have or are at increased risk for chronic physical, emotional, behavioral and developmental health conditions. The medical and social needs of CSHCN often require a high degree of support.
HIP teams may include nurses, social workers and parent professionals.
