Request for Proposal - AACO Ambulatory/Outpatient HIV Medical Care Services The AIDS Activities Coordinating Office (AACO), a Unit under the Division of Disease Control within the Philadelphia Department of Public Health, is seeking applications from HIV experienced non-profit agencies in the 9-county Philadelphia eligible metropolitan area, to deliver Ambulatory/Outpatient HIV Medical Care for Persons Living with HIV/AIDS. Applicants should attend a technical assistance conference to be held on March 28, 2008 at 1pm at 1101 Market Street,11th floor. Applicants must submit a Letter of Intent no later than April 2, 2008. The Application Deadline is no later than 12:00 o'clock Noon, Monday, April 21, 2008. Late or incomplete proposals will not be accepted. Eligible applicants are non-profit organizations that are currently funded by AACO for this service with satisfactory past performance or new organizations who serve a significant number of HIV patients and have demonstrated HIV expertise as defined in the RFP.

Request for Proposal - SAFE Program (Services to Assist Families to Excel) Supplemental Q & A Document Supplemental Q & A Document 2 The Division of Maternal, Child and Family Health, Philadelphia Department of Public Health, is seeking qualified applicants to provide services through its SAFE Program (Services to Assist Families to Excel). SAFE is a case management and home visitation program that facilitates services and provides in-home support. SAFE seeks to improve compliance with post-partum and routine pediatric care. Home visitors provide parenting education and support, help women access community resources, and provide personalized care depending on individual needs. Five one-year service contracts will be awarded through this RFP, to begin July 1, 2008.
