Traffic Accident Reports/Photographs


Please note changes in eligibility requirements for obtaining copies of Traffic Accident Reports.

The following persons are eligible for copies of reports:

  • Individuals involved in an accident
  • Automobile Insurance Companies
  • Lawyers representing individuals involved in an accident
  • Other Governmental Jurisdictions

Forms

Traffic Accident Reports may be obtained in-person or by mail.

In-person
Copies of reports may be obtained in the Records Department, Room 168 City Hall, Monday through Friday, 8 AM to 2 PM.


Mail

Requests for copies can be mailed to: Records Department, Room 168, Philadelphia PA. 19107, ATTN: Accident Reports.

Fee
For each report requested, enclose a stamped, self-addressed envelope along with a non-refundable fee of $25.00, Personal checks are not accepted. Acceptable forms of payment are cash, money order, business or bank check payable to the City of Philadelphia. When requesting copies via postal mail, please do not mail in cash.

Traffic Accident Photographs
Photographs of the accident scene, if available, may be obtained by submitting a request on the same form number 82-23. Only accidents involving serious injury or death are photographed. In all cases, the entire set of photographs taken at the scene must be purchased.

The fee for photographs is $27.00 for the first print, $9.00 for each additional print.
