To file a claim against the City, you must complete a claim form. The Office of Risk Management must receive your completed form within six months of the date of the incident. If your form is not completed and received within six months, you will be barred from bringing a claim against the City.
Follow the instructions carefully and provide all necessary documentation to support your claim. Sign and date the claim form. You can return your form to Risk.Management@phila.gov. You can also mail it to the Office of Risk Management, 1515 Arch Street, 14th Floor, Philadelphia, PA 19102.
