Complaints against businesses not following COVID-19 protocol

Update – August 13, 2021. With a new mask mandate in place to combat the spread of the Delta variant, businesses and institutions that do not require everyone who enters to be vaccinated must require everyone (employees and patrons) on-site to wear a mask. Also, masks are required at all non-seated outdoor events in Philadelphia with more than 1,000 attendees. If you encounter a business not adhering to this current mandate, let us know and we will send a request to the Department of Public Health to investigate. When calling, please have the name and exact address of the business as well as a full description of the issue. You can also use the “Business Violating COVID Protocols” service request on our mobile app or web portal to report this.

Vaccine center complaints

You can contact 311 if you have a complaint regarding a vaccination center. This includes City-run as well as non City-run clinics. Your complaint will be forwarded to the Department of Public Health. Please call 311 and an agent will assist you.
