Office of Homeless Services Data Requests

Use the forms below to request data from the Office of Homeless Services (OHS), including:

  • Reports by program type (Affordable housing, Rapid rehousing, Emergency shelter, Transitional housing, Street outreach, etc.).
  • Institutional Review Board information (Consult a full list of IRB requirements and IRB forms).

If you want to request individual-level data from multiple city agencies (including OHS), please email the Office of Integrated Data for Evidence & Action (IDEA). The IDEA staff can assist you with the multiple city agency process.
