We are pleased to share the Office of the Youth Ombudsperson’s (OYO) 2025 Annual Report. This report reflects a period of growth, transition, and continued dedication to improving outcomes for youth in placement across Philadelphia.

As of May 2026, the OYO became an independent office under the Philadelphia Home Rule Charter, marking an important milestone in strengthening our ability to serve and support young people. Throughout 2025, our team spent time visiting group homes, interviewing facility staff and meeting directly with youth, to better understand their experiences, concerns, and perspectives. This Annual Report highlights key accomplishments and activities from the past year, including casework, complaint trends, community outreach efforts, and insights gathered through facility visits and youth engagement. The report also provides an overview of the data collected and how those findings have informed recommendations, strengthened practices, and supported systems-improvement efforts.

In addition to this annual report, the OYO published two case reports, and two data reports this year. We encourage you to explore these publications to learn more about the issues impacting youth in placement as well as the observations and recommendations that emerged from our work throughout the year.
