With the closing of Hahnemann University Hospital, former patients of Hahnemann and Drexel Medicine will need to provide their medical records to their new health care providers. Here’s how to make an easy transition.

Former Hahnemann Patients

If you have a new doctor and need a copy of your Hahnemann medical records, you must complete and sign an authorization form, and then mail it to your Hahnemann practice. You can download the form from the Hahnemann University Hospital website.

If you prefer to have the authorization form mailed to you, please call 215-762-7680.

Mail the completed form to:

Please include a copy of your driver’s license or other photo ID.  Please do not fax.

Once you provide Hahnemann with a completed release form, Hahnemann will send a copy of your records to your new physician’s office.

Former Drexel Medicine Patients

If you have a new doctor and need a copy of your Drexel medical records, you must complete and sign an authorization form, and then mail or fax it to your Drexel Medicine practice. You can download the form from the Drexel Medicine website.
