Filed alphabetically by address. Each property complaint file typically contains form 97-1 (FHC "Statement of Tenant") and 81-445 (Licenses & Inspections "Housing Report to FHC"); Form 97-1 indicates type of complaint, address of property concerned; name, address, telephone number and signature of complainant, date of complaint; type of dwelling (number of rooms and baths, and whether single or multi-dwelling unit); number of occupants, adults and children; name of landlord and agent and addresses and telephone number of both; amount of first rent charged on property, amount of current rent, amount of proposed increase; name of person to whom rent paid; services provided with rent (normally utilities); indication of whether there is a written lease, its type and terms; length of time tenant has occupied property; description of condition of accommodations (usually by Licenses & Inspections; see accompanying form 81-445); indication of whether tenant has received notice to vacate or been evicted of locked out; additional comments by tenant; indication of other City Agencies to which complaint referred; names of hearing commissioners, hearing date, Fair Housing Commission "findings and orders", which usually include main points in debate between landlord and tenant.

Form 81-445 lists property address, name and address of owner, date of inspection request and inspection; Fair Housing Commission hearing date; point-score total in re "fitness" of dwelling and housekeeping units in property; types of utilities available to it; lists of orders issued to correct violations as a result of L&I inspection; L&I's Housing Section Supervisor's signature and ate; note of date of reinspection, and note of whether or not orders complied with; Housing Section Supervisor's signature and date; also included are (often) copies of form 97-3, Fair Housing Commission official order bearing property address, name of owner, date of order, Fair Housing Commission hearing docket number, specifications of order (normally an order not to increase rent until premises made to comply with City Fire and Housing Codes); also often included are tenant's initial complaint letter, copies of Fair Housing Commission's letters to L&I, to owners, to tenants; and letters, memos, from other community groups or city agencies interested in the case.
