Vols. April 1893-June 1904 add inmates' dates of death.
Vol. July 1899-November 1901 includes only women inmates, and lists only name, date of admission, address from which picked up by ambulance (or number of police patrol that delivered patient), valuables or property deposited, signature of Hospital employee receiving property, and Hospital ward referred to. Inmates' Accounts list inmate's name, date of admission, amount of cash deposited, and amount paid out at inmate's request or for funeral expenses, and monthly balances.

Vol. April 1887-December 31, 1917 is divided into male and female births and for each birth lists child's name, date of birth, race, number of the birth of that sex during the year and, beginning with January 1990, social state of mother (i.e. married or single) and Hospital ward (Venereal, Insane, Medical) in which mother confined.
Volume October 1, 1914-January 1920 lists for each birth the number of the birth during the reporting period; child's name, sex, race, date and time of birth, legitimacy or illegitimacy, and whether child was born living or stillborn; mother's full maiden name, age, number of children and number still living at the time of this birth, and residence; and name of attending physician. Beginning in January 1919 there are added monthly subtotals of male and female births and male and female stillbirths.

Generally there is included patient's name, age, race, sex, birthplace, date of admission, ward and bed numbers, dates of death and autopsy (including number of hours after death that autopsy takes place), names of attending and resident physicians, cause of death or clinical diagnosis, findings.
Vols. April 1890-March 1893 add patient's occupation and marital status, and type of weather at the time of autopsy.
Vols. March 1893-January 1920 add hour of death and autopsy, name of physician ordering autopsy, pathological diagnosis, pathologist's name and signature.
Vols. July-September 1904 and June 1908-March 1914 include post mortems performed at the University of Pennsylvania for "Blockley." Cases in these records include an Pathological Library cover sheet which includes autopsy number, date, clinical diagnosis, bacteriological diagnosis, anatomical diagnosis, and histological diagnosis; an Autopsy Protocol and Histological Protocol which add an autopsy number and the name of the person performing the protocol but omit patient's birthplace ( 1904 volume omits almost all information concerning patient.
Vols. April 1890-January 1920 add case number.
The Card Index to this Record Series is in two formats: a lesion index which lists type of lesion and autopsy case numbers where instances may be found and a numerical and chronological index which includes autopsy number; patient's name, race, sex, marital status, and birthplace; ward in which a patient, date of autopsy, volume and page of post-mortem volume where autopsy may be found, special notes (which is usually doctor's name), clinical diagnosis, and pathological diagnosis.
