Filed by parents or teachers with the Guardians of the Poor in order to have "feeble minded children" placed in the Pennsylvania Training School for feeble minded Children in accordance with Ordinance approved December 31, 1862. Each application includes identity of person making application; name, residence, birthdate, and birthplace of child; name of parents, indication whether parents are living list of diseases affecting child, indication whether there are parents' pecuniary means for support of child, source of child's maintenance in past, father's occupation; child's vicious habits, if any; peculiarities of case, signature of person or organization providing information, and date of application and approval by Board of Guardians.
