O Programa PEACE auxilia idosos com necessidades complexas de saúde física ou comportamental e que estão em situação de rua ou em risco de ficar desabrigados.

A equipe trabalha com agências parceiras para ajudar as pessoas a encontrar moradias sustentáveis que atendam às necessidades dos participantes nos ambientes menos restritivos e mais inclusivos.

PEACE significa Partners Estabelecendo Habitação Acessível e Acessível: Cuidando de Idosos Frágeis com Empatia. O programa é administrado pelo Gabinete de Serviços aos Desabrigados (OHS).

Eligibility

The PEACE Program is open to older adults (typically over age 55) who reside in homeless or behavioral health programs. Eligible individuals need:

  • A higher level of care such as personal care, long-term care, or community homes with waiver services.
  • Help with Activities of Daily Living (ADLs), such as bathing, dressing, eating, transferring, toileting, continence, or ambulation.
  • Help with Instrumental Activities of Daily Living (IADLs), such as shopping, meal preparation, housekeeping, laundry, medical management, and finances.
  • Support for complex or chronic medical and behavioral health needs.

Process of referral

Eligible individuals must be referred to the PEACE Program by case managers or support staff who work:

  • In facilities for people experiencing homelessness, such as shelters and Safe Havens.
  • In OHS or the Department of Behavioral Health and Intellectual disAbility Services’ (DBHIDS) housing programs, when individuals’ need for medical care has increased.
  • With individuals experiencing homelessness or behavioral health challenges in special circumstances as determined by OHS and DBHIDS. (Examples include staff who work with individuals who have been released from prison, discharged from hospitals, or are sleeping outdoors.)

Submitting a referral

Gather the individual’s basic demographic and health information.

Send the individual's medical evaluation forms and behavioral health notes to OHS.

You’ll need to submit the following documentation:

Scan and email the files to ohspeace@phila.gov with the individual’s initials in the subject line.
