Industrial Wastewater Discharge Permit Application is required for the issuance of a Industrial Wastewater Discharge Permit. All Users with an active Wastewater Discharge Permit are required to resubmit this form 180 days prior to the expiration of their Permit.
Groundwater Discharge Permit Guidance and Application contains guidance for completing a Groundwater Discharge Permit Application as well as a checklist to be completed and submitted along with application information.
Hauled Wastewater Discharge Permit Application must be completed and a permit issued before discharging hauled wastewater at the Philadelphia Water Department’s Southwest Water Pollution Control Plant. At this time the Philadelphia Water Department only allows the discharge of hauled septage.
Sewer Rental Factor (SRF) Application must be fully completed and submitted with the associated fee prior to the activation of an SRF. SRFs are effective the date the completed form and fee are submitted to the Department.
Guidance Documents
General Pretreatment Guidelines identify minimum specifications for pretreatment systems for commercial customers. The guidelines include but are not limited to grease trap, oil/water separator, and lint trap specifications. Exact requirement are issued on a project-by-project basis when obtaining a plumbing permit from the Plumbing Unit of the City’s License & Inspections Department.

Swimming Pool Guidance (Residential) – identifies how to properly dispose of pool water in the City of Philadelphia. The guidelines include instructions for discharging pool water to the City’s sanitary sewer system as the discharge of pool water to a storm sewer or to a water body is strictly prohibited.
Residential Oil and Grease Disposal Guidance (Residential) – provides information on how to properly dispose of used cooking oil and grease. Oil and grease should never be poured down the drain.
Permit Reporting Forms
Spill & Slug Control Plan Review Checklist (ASPP/SCP) identifies the minimum requirements for a satisfactory ASPP/SCP pursuant to Part IV, Section (3) of an Industrial User’s wastewater discharge permit.
Semi Annual Compliance Report Cover Page may be used by permitted Industrial Users submitting their Semi-Annual Compliance Report to the City. Please ensure when sending your report that you include all additional information as required by the City.
Industrial Wastewater Discharge Signatory Designation Form (Industrial Wastewater Discharge Permit) may be used by an Industrial User establishing an authorized representative responsible for reviewing and signing reports required by the User’s wastewater discharge permit. Carefully review the attached instructions and necessary conditions under which a person may be designated as a signatory of an Industrial User’s reports.
Groundwater Discharge Signatory Designation Form (Groundwater Discharge Permit) may be used by an Industrial User establishing an authorized representative responsible for reviewing and signing reports required by the User’s groundwater discharge permit.
Name or Ownership Change Notification Form may be used by an Industrial User notifying the City of an upcoming change to a regulated facility’s name or ownership. This notice must be submitted to the City at least 30 days prior to any change.
Closure Form (Industrial Wastewater Discharge Permit) may be used by permitted Significant Industrial Users to aid with submission of a complete permit closure statement. A closure statement must be submitted to the Industrial Waste Unit at least thirty (30) days prior to closure pursuant to Part IV, Section (5) of an Industrial User’s wastewater discharge permit.
Closure Form (Groundwater Discharge Permit) may be used by permitted dischargers of pumped groundwater to aid with submission of a complete permit closure statement. A Closure Statement must be submitted to the Industrial Waste Unit at least thirty (30) days prior to closure, unless otherwise stated in the “Reporting Requirements” section of a groundwater discharge permit.
Compliance Forms
One-Time Compliance Form for Dental Dischargers is used to verify that the facility is compliant with the pretreatment standards (40 CFR Part 441, Dental Office Category). An authorized representative of the facility must complete a one-time compliance form and submit it back to the City of Philadelphia’s Water Department Industrial Waste & Backflow Compliance Unit.
