Rowan Homes Judson Street/Diamond Street Philadelphia, PA 19121 Services Provided: Rowan Homes Judson Street, the first phase of the project, offers 32 two- and three-bedroom apartments, as well as community space, a quiet courtyard and a playground. Rowan Homes Diamond Street, the second phase, offers 8 four-bedroom apartments for larger families, as well as community space and a quiet courtyard. Additional services include case management, youth and adult education and job training. In Community 1229 Chestnut Street Philadelphia, PA 19107 Services Provided: clean and sober, supported independent living program. It provides 32 affordable, semi-furnished, efficiency apartments to formerly homeless men and women dealing with mental health issues who require regular, but not around the clock supportive services and supervision. St. Columba 4133-39 Chestnut Street Philadelphia, PA 19134 Services Provided: The 25 Safe Haven Beds are targeted for those men just coming off the street and in need of close supervision and support. Services are provided on a 24-hour basis, including case management, support services and on-site medical care. The 15 Single Room Occupancy (SRO) Permanent Supportive Housing units are for chronically homeless men with disabilities who are able to perform daily living tasks and can provide 30% of their income towards rent. TRANSITIONAL HOUSING Kairos House 1440 N. Broad Street Philadelphia PA 19121 Services Provided: Provides affordable SROs to formerly homeless adults who require regular supportive services and supervision. Residents work intensively with in-house case management services to establish and attain goals outlined in their service plan. Kairos House services include meal preparation, medication management, financial management and 24 hour staff coverage. Residents provide 30% of total income towards room and $90 a month for food. St Elizabeth?s Recovery House 1850 North Croskey Street Philadelphia, PA 19121 Services Provided: 26 single room occupancy units for men who are homeless and have a substance use disorder with or without a co-occurring mental illness. The program is structured and requires resident readiness to change substance use behaviors. Residents pay 30% of their income for rent and $20 per week for food.
