How to Get a TEEN CENTER I.D.

All forms are available at any of the Teen Centers. See the supervisor or Outreach Worker. You must have ALL of the following:

1. A Completed Consent Form

If you are under 18 years old, you and a parent/guardian must sign it.

If you are 18 or older, you must sign it.

2. A Completed Emergency Form

If you are under 18 years old, you and a parent/guardian must sign it.

If you are 18 or older, you must sign it.

3. A Completed Registration Form

Signed by you

4. Proof of Age Using One of the Following:

A school I.D.

Birth Certificate

Green Card

Drivers License

Return all forms to the supervisor or an outreach worker at your Teen Center.
