HOW TO GET A TEEN CENTER I.D. CARD

All forms are available at any of the Teen Centers. See the supervisor or Outreach Worker.  You must have ALL of the following:

1. A Completed Consent Form

          o If you are under 18 years old, you and a parent/guardian must sign it.
          o If you are 18 or older, you must sign it.

2. A Completed Emergency Form

          o If you are under 18 years old, you and a parent/guardian must sign it.
          o If you are 18 or older, you must sign it.

3. A Completed Registration Form

          o Signed by you

4. Proof of Age Using One of the Following:

          o A school I.D.
          o Birth Certificate
          o Green Card
          o Drivers License

      Return all forms to the supervisor or an outreach worker at your Teen Center.
