RECREATION DEPARTMENT MANUALS AND REGULATIONS
EMPLOYEE ORIENTATION MANUAL
Safety and Injury on Duty
For your own protection, and that of your fellow employees and the general public, employees are required to observe safe work practices as defined by City, Department and Unit rules, as well as by the standards of your occupation. Your supervisor will instruct you in the proper safety practices of your position. If an employee is not sure what they are, he should ask for guidance. An employee is required to wear and use all appropriate safety clothing and equipment issued to them.
If you are injured on the job, report the injury to your supervisor immediately, no matter how slight it may be. If your injury requires emergency medical treatment, call 911 or go to the closest hospital for treatment. If your injury is less severe, you should report as soon as possible to the health care provider listed on the following page which covers the zip code where you are assigned. In order to be treated, you must bring with you a copy of Form 82-S-30 (Industrial Medical Care Provider Authorization.) signed by your supervisor. If you suffer a work related injury you are required to treat the injury with a city approved physician or health care provider for a period of thirty days starting with your initial visit.
You must also fill out an official City of Philadelphia Injury Report Checklist form, available at your worksite or through the Safety Office, and file it with the Safety Office.
After treating with a City approved health care provider for the initial 30 day period following your first visit, you are permitted to treat your injury with a health care provider of your own choice. The City will be responsible for payment of reasonable surgical/medical services, and medicines and supplies. You must notify the City's Risk Management Office in writing of this treatment within five days of your first visit to your chosen health care provider.

Injury Leave - Injured on Duty (IOD) Benefits
An employee who is injured on the job and disabled as a result of the injury is eligible for Injury on Duty (IOD) benefits. Under the City's disability program (Regulation 32), he will receive 75 % of base pay after taxes in lieu of Worker's Compensation, and he may continue to earn sick time while recuperating from his injury.
To qualify for coverage under the City IOD plan, an employee must immediately report an injury to their immediate supervisor, and agree to be treated at an authorized health care provider. Treatment must occur within 24 hours of injury. Injury reporting forms are available at your worksite (82-S-58).
Employees must receive a referral for evaluation from their supervisor and treat at one of the designated City of Philadelphia provider sites. The zip code of the facility/location where the work accident occurred determines the hospital or medical office that will treat the employee.
