This record series includes four different types of volumes used for recording births at the Hospital.
Records of Births, January 1920 - March 3, 1959, include the number of the birth during the reporting period, child's name, sex, race, date and time of birth, legitimate or illegimate, and whether child was born living or a stillbirth; mother's full maiden name, age, number of children and number of children still living at the time of this birth, and residence; father's name, age, and residence; name of attending physician; and monthly subtotals of male and female births and male and female stillbirths. Beginning in 1942 there are included Receiving Ward births and births outside of the Hospital.
Birth Records, April 1, 1959 - March 31, 1961, are a group of four composition books which list baby's name, date of birth, first name of mother, and notations if childn was born outside of the Hospital or in Receiving Ward. There are occasional notations if the mother of the child is single.
Records of Birth, April 1, 1961 - September 30, 1976, are entered on plain loose leaf sheets or, beginning with September 8, 1968, on form 55-G-1684, "Birth Record and Birth Certificate Receipt," which lists date of birth, baby's name and birth weight, mother's maiden name and (beginning with April 1, 1968) place of birth and, from September 18, 1961, number of birth of birth on that date. Entries April 1, 1961 to July 4, 1966 add Hospital representative's signature or initials and date signed. From July 5, 1966 to September 30, 1976, the records add the date the birth certificate was sent, the signature (presumably) of Hospital official who received the certificate, and date of receipt. Entries November 23 to December 10, 1967 include only the date of birth, baby's name and first name of mother of baby.
Register of Births, December 27, 1952 - July 14, 1957 is indexed by child's name and includes the number of birth, child's name, sex, date and hour of birth; and indication if birth was single; father's full name, race, age, birthplace, usual occupation and business or industry in which employed; hospital number; mother's full name, residence, color, age, and birthplace; number of children previously born to mother, number of her children living and number stillborn. The Registers of Birth also include the following additional information: name of attending physician, December 27, 1952 - January 4, 1957; date certificate signed, April 27, 1954 - January 10, 1957; legitimacy or illegitimacy of child, March 9, 1954 - December 27, 1956; and, beginning with January 1957, notations of low birth weights or premature births. Volume December 27, 1952 - January 17, 1955 includes a typed "Daily Routine on Birth Ceritificates."

Includes autopsy or post-mortem number; the deceased's name, age, sex, race, and date and hour of death; date and hour of the autopsy, the name of the physician performing the autopsy, and autopsy findings.
Entries January-March 1920 add deceased place of birth; and case, ward, and bed numbers.
Entries January-July 1920 add name of physician who ordered autopsy, clinical diagnosis, bacteriological diagnosis, gross anatomical diagnosis, histological diagnosis and include an autopsy protocol and histological protocol which include post mortem number, name of doctor who prepared report, and description.
Beginning in July 1920 the post mortem records replace the Clinical, bacteriological, gross anatomical,, and histological diagnoses with a clinical abstract, results of internal and external examinations, histological findings, and, occasionally, pathological notes and /or microscopic extracts and diagnosis. There are also occasional photographs of tissue samples. Many cases are coded to indicate "well worked up" or particularly interesting cases. Some records March 1940-January 1966 add the name of the physician in service at the time of autopsy, a gross anatomical description, anatomical description, microscopic description, and often patient history. Many of the post mortem examinations, 1940-1966, seem to have been performed at Philadelphia General Hospital for the Women's Medical College of Pennsylvania.

Vols. August 1922-March 1923 adds person to whom report is directed.
Vols. beginning September 1966 include patient's address.
Vols January 1972-June 1977 omit most personal information concerning patient but add a fiscal code, important features of case history, operation, and tissue slide numbers.

Personnel files for doctors, dentists and other medical personnel who served internships or residencies at Philadelphia General Hospital. Although the files date from as early as 1931 the bulk of the files seem to cover the period from ca, 1948-1977. Contents of the files vary from period to period and on the basis of service at the Hospital. Items found in the folders include:

Application for Internship which lists applicant's name, home and mailing addresses, telephone numbers, marital status, date of birth, height, weight, physical deformities, educational background, past medical history; whether applicant earned his way through school; his record of educational scholarships, extracurricular activities; business, professional, and vocational activities; research problems carried out, publications, names of family members in medicine, and signature;

Performance Ratings by chief resident which lists chief resident's name, date, name of intern or resident,, identification of service, evaluation for each category, suggestions or remarks, signature of chief resident and of medical director;

Internship Record listing doctor's name, birth date, addresses, colleges and medical schools attended, date reported and completed, reason for absences, schedule of services, internship hospital, and date of Pennsylvania license;

Application for Residency which lists date, subject of residency; applicant's name, military status, home and present addresses, marital status, date of birth, nationality, high school, college and medical school attended and dates, hospital and dates of internship, description of past graduate study and medical practice, scholarships, extracurricular activities, description of scientific research; and the names of references;

Resident Contract or Contract for Appointment To Civil Service Exempt Position; House Staff Personnel Form listing parent hospital, area of training, date of start and finish at Philadelphia General, graduate physician level; applicant's name, present and permanent addresses, sex, birth date, place of birth, phone numbers, social security number, marital status, citizenship, present medical or surgical status, educational background, professional experience, medical practice, licenses to practice and how obtained; and names and addresses of spouse or nearest relative;

House Staff Record which lists name of member of house staff , address, current position, when reported and when completed, college and medical attended, hospital of first year graduate training, number and date of Pennsylvania license, field of graduate training military status, and schedule of services; roster cards which include subject'' name, date of birth, residence, social security number, salary range; name, address and telephone number of reference; civil service status, date of appointment, job title, salary, department in which employed, remarks, payroll number, and notations; copies of letters from Secretary of Board of Trustees of the Hospital announcing appointment as resident, copies of letters of recommendation prepared by the Director of Medical Education, record of intern rotation, verifications of internship or residency at Philadelphia General Hospital, and documents relating to foreign interns or residents.. One cubic foot of records consists of reels of microfilm of existing files and those which no longer exist with manuscript indices.

Items to be found in the files include Medical Staff Appointment and an earlier form , Application for Appointment, Medical Staff, Philadelphia General Hospital, which list division or department of the Hospital to which application is being made, applicant's name, age or birth date, residence, medical school at which applicant obtained medical degree and year of graduation, institutions at which applicant served internship and residencies and dates and present hospital staff appointment or experience and hospital affiliation, Pennsylvania medical license and date issued, date and signature of applicant, and the following omitted on one or the other of the forms: telephone number, expected schedule and dates of service, Federal narcotics number; dates and signatures of chief of service and representatives of Credentials Committee of Medical Staff, Executive Committee of Medical Staff, Executive Director or Secretary of the Board, and Board of Trustees ( omitted on earlier form) and indication of membership in County Medical Society and state and date of license in state other than Pennsylvania (omitted in later form); a Supplement to the Medical Staff Appointment Application form which includes applicant's name, sources of funds, amount paid, remarks, date and signature of coordinator, and school division, Request for Medical Staff Changes of Title/Schedule form which includes name of Medical Staff member ,service and school where service performed, title change, change in schedule, salary from Philadelphia General Hospital, reason for change in title or schedule, effective date, and date and signatures of Chief of Service/ Department Chairman, Director of Credentials Committee, representative of Executive Committee, Executive representative of Board of Trustees; letters of appointment, acceptance of appointment, resignation, and complaint; physician's curriculum vitae of biographical sketch, copies of news clippings or other publications related to a specific Staff member, and memos.
