Topics include municipal subsidization of voluntary mental health clinical establishments, agreements with the hospitals, requests for clinic funds, disposition of individual cases by the hospitals, procedures, and reports on hospital clinical facilities. Also included are minutes of meetings of the Division of Mental Health, Community Health Services staff for the dates April, May, October 1967 and minutes of the Advisory Committee of the Voluntary Health Clinic Program, December 14, 1961.

Under heading of each participating mental health clinic are listed the patient's name, the date case file opened, patient's application number, fee paid by patient, number of treatments received each month, code for type of therapy received (including diagnosis, therapy, psychological testing, or therapy with mother, father, parents, wife, husband, family therapy, therapy with minister, or family diagnosis), date case file closed, and code for case closing (improved, diagnostic evaluation and/or consultation only, referred to other sources, ineligible, poor diagnosis/unimproved, broken contact with patient, lack of psychiatrist, approved but never opened, left city/died, modification for presenting problems, or clinic closed).
Vols. 1961-1964 add amount payable by City for each month's treatments.
Vols. 1962-1963 add fund allotment to each institution.

Department of Public Welfare Form-MH/MR 10. Each form lists date, case number, degree of intervention priority, patient's sex and race, age, date of birth, level of education, marital status, occupation or employment status, income, religion, number of dependents, previous mental health or mental retardation services, intake evaluation and analysis, description of proposed service plan, indication if base service unit or other institution will provide service, monthly payment for service, liability for payment, signature of base service unit physician. Often attached to the forms are a "Mental Health and Mental Retardation Program Authorization for Service" (Department of Public Welfare Form-MH/MR 11), which lists case number, name and address of institution providing service, administrator's signature and date, service to be provided and maximum amount of monthly and daily payments, name and address of patient, patient's base unit (which is often the same as institution providing service), amount of payment by County Mental Health and Mental Retardation Program, by patient or third parties. Another attachment often found is "Mental Health and Mental Retardation Program, Service Rendered Report". Twenty-one cubic feet of these forms have been keypunched as a statistical sample. Keypunched forms add source of referral, date of disposition of case, form of disposition and reason.
