1.      Completed three-part HRP application forms (the Development Team (2 copies)., Property Information, and Acquisition and Appraisal Information). (1 copy) Please include the federal Employer Identification Number, (EIN) and the contractors license number.                    _______

 

2.      Evidence of general liability insurance for the contractor.  Additional evidence of insurance specific to the project will be required later in the process.                                             _______

 

3.      Evidence of site control, deed, agreement of sale, or memo from the RDA for properties in public inventory. A settlement sheet is preferred.                                                                     _______

 

4.      * A block survey: The RDA form is required.  If you have a map or better form you can submit it in addition to the RDA form. Please make sure you follow the symbols specified in the legend. Please identify all vacant buildings and lots, the subject properties, and the ownership information for all vacancies.  You must indicate the total number of properties on the block, the number of vacancies, and the calculated future vacancy rate.                                     _______

 

5.      * A budget, the RDA form is preferred.                                                                       _______

 

6.      * Construction documents  A work write for projects costing less than $50,000, drawings and a trade cost breakdown for projects costing over $70,000, and a judgement is made of projects costing between $50,000 and $70.000.  Please attach a schedule of values or pay-out form as a first page)      (3 copies, 4 copies if the property is historic)

 

7.      A Lead Hazard Risk Assessment prepared by a state certified Risk Assessor  (2 copies).______

 

8.      An after-rehab appraisal from a state certified appraiser. This can be submitted later in the process to coordinate with the construction lender, but must be submitted before the RDA can issue an approval letter. If the property costs more than $15,000, the as-is value must be included in the comments of the after-rehab appraisal.                                                           _______

* If you want a “Preliminary Review”, this is a quick review that will provide you with a limited approval of the project, please submit the starred items.  This “preliminary approval” will give you, or an interested contributor or lender, confidence that once you make application it will be accepted and processed.

For construction documents, for a preliminary review only, please supply a one page cost estimate. 

Anarrative description of the project would also be helpful, at minimum the name, of the developer and contractor.  References would also be helpful.
