The Nurse-Family Partnership is a program for first-time, low-income mothers. A public health nurse provides home visits to mothers every one to two weeks during their pregnancy. The home visits continue until the child is two years of age. The nurse home visitors focus simultaneously on the mother's personal health and development, environmental health, and quality of care giving for the infant or toddler. The nurses involve family members and friends in the program and help families use other community resources when needed.
The Nurse-Family Partnership Program is a collaboration between the City of Philadelphia Departments of Public Health and Human Services, LaSalle University Neighborhood Nursing Center, MCP/Hahnemann University, Temple University and the Visiting Nurse Association of Greater Philadelphia.
