The Health Federation of Philadelphia's Training and Resource Center provides training to community educators, outreach workers, home visitors, interpreters and nurse supervisors providing direct services. The Center provides ongoing general orientation, technical assistance, consultation services, audio-visual resources and peer support. |

Hospital of the University of Pennsylvania |
This program provides intensive outreach and interconceptional case management services to at-risk pregnant and post-partum women and their infants/toddlers up to six months of age. Targeted clients are those who are identified as at-risk for compromised medical, health and/or psychosocial outcomes upon their entry for perinatal health care services due to presenting inadequate levels of prenatal, postpartum and pediatric care, as well as psychosocial and/or environmental risk factors. |

Intercultural Family Services |
This program provides outreach south of Market Street in the neighborhoods of Cobbs Creek, Paschall, Kingsessing and University City (zip codes 19104, 19139 and 19143). An outreach team identifies at-risk women and infants, promotes the importance and availability of perinatal health and social service resources, and ensures at-risk women and infants are connected with necessary prenatal, postpartum, interconceptional and pediatric services. |

Maternity Care Coalition |
The MOMobile is a family support program that uses targeted outreach to provide education, referral and follow-up to high-risk pregnant and parenting women and their families. Through door-to-door canvassing and community outreach, trained community health workers (advocates) help to identify pregnant and parenting women and assist them in beginning prenatal care or following through with post-partum and baby care. Advocates help women to keep appointments and obtain additional services through home visiting, on-site advocacy, and group support activities. They support clients through their pregnancy and the first year of the baby's life. MCC also conducts parent outreach meetings on such topics as: family planning, HIV/AIDS, lead poisoning prevention, breastfeeding and safe infant feeding practices, community resources, self-esteem and self-help. |

Mayor´s Office of Community Services |
The Mayor's Office of Community Services allows pregnant and parenting families in West and Southwest Philadelphia to borrow items that are useful in caring for their children, including: playpens, strollers, car seats, cribs, highchairs, bottles, diapers, clothing, blankets, toys, etc. Large items are loaned for a period of up to one year; smaller items (diapers, clothing, toys, etc.) are given out. The Community Exchange also assesses the health and social services needs of at-risk pregnant and postpartum women and their infants. The program helps to enroll them in appropriate care, and connects them to outreach services for further support. |

Supportive Child Adult Network (SCAN) |
In collaboration with the Southeast Asian Mutual Assistance Associations Coalition (SEAMAAC), staff provide case management services to improve the health of perinatal/interconceptional clients who are at-risk for poor health outcomes due to health, environmental and psychosocial risk factors. The North Case Management Team serves Haddington/Overbrook and Mantua/Millcreek/Parkside. The South Case Management Team serves Cobbs Creek, Eastwick/Elmwood, Paschall/Kingsessing and University City. The Interconceptional Care/Case Management Team serves clients in neighborhoods throughout the project area. |
