A HELPING hand
Since 1998, the Health Intervention Program (HIP) for Families has provided a helping hand to Philadelphia families who need extra support caring for their children and adolescents.
A PERSONAL care plan
HIP Families helps parents and caregivers cope with their children's day-to-day health problems before they become overwhelming.
HIP Families works with families in their own homes to help them design a personal care plan for their children.
Each care plan includes: a complete home health assessment, hands-on health education, and linking families with practical resources they need.
A TEAM approach
Each HIP Families team includes: nurses, lay home visitors, social workers, nutritionists and experts on breastfeeding, substance abuse and child development.
