Completing centralized city-wide registration and eligibility determination
for individuals seeking services through the mental retardation system.
Working in partnership with 12,000 individuals and their families
to assist in supporting the individual to have the services needed.
Administering services and supports through 70 contract agencies to:
4,000 children and families using early intervention services, 2,500
people living at home and using day and employment services, community
resources, family support services, respite care, therapies, social
and recreational services, and 1,740 people living in supervised residential
homes in the community. Collaborating with the education, health,
and child welfare systems to serve children from birth to 21 years.
Collaborating with vocational rehabilitation, health, and aging to
serve adults. Direct responsibility for provision of court ordered
supports coordination for 519 individuals who are living in the community
who are Embreeville and Pennhurst class members. Planning and identifying
the needs of 3,000 individuals living at home who are on waiting lists
for support services, day and employment, or residential services.
Monitoring to ensure a focus on Quality Management and accountability
for service outcomes.
