• Fee is $35.00.
• Allow 10 days for report.

Fire and/or Emergency Medical Services (EMS)

Apply in person at the Police Records Unit or by mail.

• Download form number 82-311 (or fill it out online and print).
• Signed authorization of the injured party MUST be given (form 82-311B).
• Fee is $20.00 (non-refundable).
• Allow 6-8 weeks for report.

Letters of Good Conduct

Apply in person at the Police Records Unit or by mail.

• Fee is $35.00 (non-refundable).
• Allow 10 days for report.
