You and/or your Life Partner may elect to terminate your Life Partnership. 

If both of you sign a Termination Statement, your Life Partnership will be terminated effective sixty (60) days from the date you and your Life Partner file a Joint Termination Statement with us.

If only one Life Partner signs a Termination Statement, your Life Partnership will be terminated effective sixty (60) days from the date an Individual Termination Statement, along with a Termination Statement Proof of Service, is filed with us.

The Termination Statement Proof of Service is a document that affirms that a copy of the Individual Termination Statement was given to the other Life Partner. You can complete service in one of two ways. You personally deliver a copy of the Individual Termination Statement to your Life Partner, or you can send a copy to your Life Partner by certified or registered mail.
